This article is part of the “Form of the Month” series, where we regularly describe a form that can inspire others. You are welcome to tell us about a form that you find inspiring – and it can be your own.
Kvinder i fodbold
Kvinder i fodbold is a new Danish organization that works for more opportunities and visibility for girls and women in football. They are working to get §8A approval, and here the form is an important element. It must ensure that they get the important 100-200-300: 300 members, 100 supporters who give at least 200 DKK.
We think that Kvinder i fodbolds membership form is sharp, and therefore it is the form of the month. You can try it (and possibly sign up) here.

Razor-sharp communication
It is especially the razor-sharp communication and clear guide to the potential member that inspires us in this form. The pitch is great because it speaks precisely to the prospective member – you are helping to create change. Here it is not what the organization can do for you, but the difference you can make – with the organization.

This is a good thing to note, because it is actually one of the crucial differences between members and donors: Members are a bit more committed and they want to have an influence. So the focus should very much be on the member and their place in the joint organization.

The form also reflects this nicely, as there is space here to both select a local branch and write a note. That element sends a nice signal, especially if Kvinder i fodbold responds. And they do!
Userfriendly response
Another reason we’re excited about the form is its user-friendly design. The top bar clearly aligns expectations – you’ll spend the most time typing in information.

Next, they have chosen a very user-friendly model for displaying price and frequency. It is not so classic for NGOs, but it is straightforward and easy to read. Of course, this requires that you have a simple model – maximum three options, otherwise users cannot decode it.
Finally, the fields are clear and easy to decode because labels are placed inside the field. You know exactly what is expected where, and it can be seen in the data quality. The address field is with DAWA-lookup, and that helps too.

On the road to approval

As mentioned, Kvinder i fodbold are working towards acclaiming a §8A-approval. This means that one-time donations over 200 DKK also holds great value for them.
That’s why their donation form will be the last thing highlighted here, because it’s easy to copy: Set the lowest amount to 200 DKK. It works!
